Care Partners Assisted Living

Sunrise of Mercer Island is like a big, warm hug. You can feel the compassion that the staff has for the residents as soon as you walk into the building, and the care provided is second to none. I take great comfort knowing that those who live here are well taken care of and happy within their home.

I would give it the same rating except a 5 for value. My sister would probably do the same. The activity rating wasn't given because Ed didn't do activities. Several times I saw them doing group activities and the residents seemed engaged and having a good time. I would put a solid 4 down for that. I would recommend Care Partners Hortonville to others.

I have had two family members who have stayed at care partners. My mom was in the assisted living building, and my dad was in memory care. The staff were amazing and provided excellent care. I was very impressed the entire stay for both of them.

Our loved one was only there for [dates removed] and during that time he received the best care. The staff members there were absolutely wonderful and the manager [name removed] she was wonderful she gets a big gold star. He had a big enough apartment that could fit a game table, bed, chair, big TV and dressers it was wonderful. They kept the community really clean and from what we could smell of the meals it seemed good. Based on our experience we would recommend this community to other families.

They had a really thoughtful and full welcoming and transition approach. They really took the time to understand what my mom needed, and they have tweaked things along the way based on what she needs. The facility is great as well. It's clean and meets her needs. There are a variety of activities and places for Mom to do things or hang out and be social. One unique feature that we were looking for and found at Sunrise Mercer Island is a secured assisted living facility. So Mom, at an early stage of dementia, still has the freedoms of assisted living and gets to interact with people who are not on the level of needing memory care. Yet the facility is still locked, which gave us peace of mind because she has some wandering behaviors. Since she is still super communicative, being able to interact with others who are not at an advanced stage of dementia has been very helpful to her.

I would recommend the community to others for their loved ones. I like the fact that the community is small, and the residents get more individualized care. It is easier for the residents to make friends. The rooms are nice size. They keep the community clean. They are receptive questions and concerns.

Started in [date removed] contacting over 10 different facilities. In [date removed] we were ready to place. Care Partners was one of the few facilities that returned calls and set up tours quickly. We toured 4 facilities and loved chose this location. A mature building nestled in a magnificent wooded area filled with wildlife and kind loving caregivers. New Director [name removed] gave me her cell phone and was great returning text the week of move in even when she was on vacation. Our Loved one has an over heating issue with their body and during our tour we asked about installing a ceiling fan to help him stay cool. The day of move it it was installed early in the morning. They allowed us to install extra grab bars in bathroom as our loved one is super high fall risk. They have been extremely helpful during moving in with questions and requests and after. As a former assistant living employee I know the industry and I would highly recommend Care Partners for their exceptional customer service.

I have been very pleased with Care Partners Assisted Living and Memory Care. We chose this facility because I was in a time crunch and they were very accommodating and receptive to our needs. The room sizes here are very spacious, more than enough room for him. The bathrooms are nice size, wheelchair friendly. They have not had any cases of covid, residents or staff so they are very diligent with their protocols. I would highly recommend them.

We looked through many places. This facility had a specific Memory Care area, private rooms, and a fixed cost so there was no monthly evaluations to increase the price. Excellent communication. Enjoyed having meals with my Dad and walking around the building. They care for Dad through a number of different medical situations and followed our direction for Dad.

Very home like feeling which is important to our family. We could see our mother in this facility. The open spaces surrounding the building was appealing. The staff was friendly as were the residents. Clean and updated decor.

My husband has lived in Memory Care at Care Partners in Hortonville the past three months. I am impressed with the kindness, patience, and expertise these Caregivers show in their respectful care of residents with dementia. The Activity Director is especially attuned to individual needs and abilities, even in a roomful of residents - a very talented self-esteem builder! All staff have been most friendly, even fun, giving this very attractive place it's homey feel.

Most pleasant place. The staff is always kind. The RN is so patient and smart. The residents are calm and happy. Always smells nice and filled with love and compassion.

An incredible community with the most lovable and caring staff. We are so glad we found this place, they are truly a blessing.

We couldn't be more pleased with our experience here at Country Terrace. [Name removed] runs a tight ship, her attention to detail is incredible. She is personable and professional, communicates well and has her residents best interests always at the forefront. We have found her team members to be long standing employees. They really seem to care about treating the residents like family - very patient and kind. Everyone is on a first name basis, it's such a relaxed and comfortable environment. My Mom has made the transition well and when my Dad visits (daily), he is made to feel welcomed.

This place is so far my first choice. The size, the bridge from assisted to memory care and personal caregiver attention are all strong. It's got more of a home feeling. The only negative is the size of the studio available--very very small

This is a great community and I am happy to see that she is very happy here with the staff and the cleanliness of the rooms and the grounds that she enjoys. The staff are always caring and helpful. The meals are appealing and she is pleased with the variety. The activiites are helpfull for her. We do recommend this community for great care for loved ones with a good value.

It was a nice tour and we were stuck between them and another place. It is a big facility for the residents. She would of had a little apartment. It is more of a community living. We were looking for more care and felt that it would not have been the best fit. I would absolutely recommend them to family and friends.

A bit pricey at over 6000 per month. On the other hand, everything is covered with that fee, and I really liked the feel of the place.

I ended up moving my Father here because he would be closer to me. This facility is still new and they aren’t filled to capacity yet. The staff is still minimal but they are very attentive. This community is beautiful and there is a nice wooded area behind the facility. They have a wonderful back yard with a large gated area for walking. They offer a lot of activities like coloring, games, and music. The only thing I wish they could do was give my dad more physical activities. I would recommend this facility to other families.

I truly can not say enough positive compliments for the staff at Country Terrace. Over the almost two years my mother has been there due to dementia, we have found the staff to be diligent, kind and very personable to not only our mother but our entire family. Myself and two siblings all live outside the area and never notify staff when we are coming for visits and always find our mother very well cared for when we arrive. I live in Washington, D.C. and find the staff willing to keep my updated on her care. I often say that I miss my mother horribly but don't have to worry about her because I know she is in excellent hands. She is treated like family.
